Is there an unrecognized epidemic of primary aldosteronism? Con.

چکیده

My answer is “no.” Although primary aldosteronism (PA) is likely more common than most experts believed in the 1960–1990 interval, it is not nearly as common as Dr Calhoun and other investigators report from 1990 until now. As many younger clinicians may not remember, this same issue arose soon after Dr Jerome Conn characterized this disease in 1955.1 Reporting on the prevalence of PA in his highly referred population of likely suspects, Dr Conn and coworkers’ estimates were as high as 20%.2 Subsequently, studies on unreferred patients supported a prevalence of 1%.3

Is there an unrecognized epidemic of primary aldosteronism? Pro.

As evidenced by this debate, controversy persists as to the true prevalence of primary aldosteronism (PA) and, perhaps more importantly, concerns regarding the clinical rationale of evaluating large numbers of hypertensive patients for evidence of aldosterone excess. In regard to the former controversy, it has been suggested that reports of a high prevalence of PA exaggerate the true value for ...

Would wider screening for primary aldosteronism give any health benefits?

Fifty years ago, Jerome Conn described 'a new clinical syndrome which is designated temporarily as primary aldosteronism' in a young patient with hypertension, severe hypokalaemia and a benign adrenocortical tumour, for whom an adrenalectomy cured both the hypertension and hypokalaemia. His report identified the condition known as aldosterone-producing adenoma, a form of curable hypertension. A...

گزارش یک مورد هیپرآلدوسترونیسم به علت Aldosteronoma

Primary hyperaldosteronism is one of the few causes of hypertension that can be cured by surgery. Primary hyperaldosteronism is caused by adrenocortical adenoma or hyperplasia. It is important to differentiate between adrenal adenoma and hyperplasia because the preferred treatments are different. In all patients with new-onest or worsening hypertension the primary hyperaldosteronism should be c...

Stability and Bifurcation of an SIS Epidemic Model with Saturated Incidence Rate and Treatment Function

In this paper an SIS epidemic model with saturated incidence rate and treatment func- tion is proposed and studied. The existence of all feasible equilibrium points is discussed. The local stability conditions of the disease free equilibrium point and endemic equilibrium point are established with the help of basic reproduction number. However the global stabili- ty conditions of these eq...

In vivo and in vitro studies of adrenal secretions in Cushing's syndrome and primary aldosteronism.

Abnormalities in aldosterone secretion are unusual in Cushing's syndrome even in the presence of hypokalemic alkalosis (1). Similarly, increased cortisol excretion is not seen in primary aldosteronism (2-7). Little information regarding corticosterone secretion is available in either condition. Increased excretion of corticosterone metabolites has been suggested in several cases of primary aldo...
